I also understand and give consent for Signatures Style Lounge to use my pictures for training purposes or in promotional material and advertisements, and will not seek financial gain for such usage.
I, the undersigned, hereby voluntarily agree to accept the services and terms noted below by any cosmetologist working at Signatures Style Lounge for the services performed and release Signatures Style Lounge and Stylist/Team Member, from any responsibility and/or liability concerning the application, processing, engagement with, and/or consequences of services rendered. I consent to receive services provided at Signatures Style Lounge.
I release and hold Signatures Style Lounge, its employees and its agents harmless against any and all liability, damage, and/or expenses arising out of or in connection with actions, claims, and/or damages resulting in personal injuries and disabilities (physical and/or psychological) that I might incur as a result of any service(s) received. I understand that additional services may be recommended and/or necessary for maintenance and that permanent damage may be possible due to any chemical/services received.
We pride ourselves on providing all clients with fantastic service and quality customer care. We are professional, highly- skilled and experienced, and will provide you with the best hair care services possible.
However, if you are not satisfied with the results of your salon service, please contact us the salon within 24 hours of your appointment. We will ask you to return to the salon for your hair to be visually assessed within 3 business days of your appointment, and are able to provide a one-time complimentary adjustment within 5 business days of your appointment date. We do not offer refunds on salon services.
